Changes in the User Identification Method upon Subscription
Please note that, in accordance with the full enforcement of the "Law Concerning User Identification of Subscribers, etc. and the Prevention of Unauthorized Use of Mobile Voice Communications Services by Mobile Voice Communications Carriers (Mobile Phone Unauthorized Use Prevention Law)" as of April 1, 2006, au has changed the user identification method for new subscribers and users.

What customers need to apply for au subscription

Credit Card + Health Insurance Card
Customers wishing to pay the monthly charges by credit card will be required to produce a credit card issued in their own name. Health Insurance Card: Must be valid with the customer's current address shown

Initial costs
Subscription Processing Fee: ¥2,700 (¥2,835 including tax) This fee will be invoiced together with the first month of charges. Cost of any purchased products (handset, battery, etc.)

Information exchange among mobile phone and PHS service providers regarding nonpaying users or senders of unwanted e-mails
Information regarding users who have failed to remit payment upon termination of a contract or users who send unwanted e-mail messages will be shared among mobile phone and PHS service providers. Such information will be used for screening when accepting applications. Accordingly, some applications may not be approved under some circumstances.

Regarding applications from students who are minors, au reserves the right to contact the parent/guardian to confirm consent.

These documents are not required if the Student ID is presented as personal identification for new subscription applications.
Important Notes about Required Documents
Applicants must present originals of the Student ID and Enrollment Certificate. Copies will not be accepted. (Enrollment Certificate must be presented within 3 months of their issue date.)
Written consent by parent/guardian must be signed and have the personal seal of the parent/guardian affixed.
We ask that the subscriber [2] visits an au Shop or PiPit (procedures may be completed at a shop other than the one in which the subscription was started).

If the subscriber is a child of elementary school age or younger, he or she must be accompanied by a parent or guardian.
Model change is possible even if the customer’s current au phone has been lost. Please bring all necessary items listed above except "Your current au phone".

A model change processing fee of 2,000 yen (2,100 yen with tax) will be invoiced together with the first month of charges following the model change.
